  • If your practice offers the HekaHeart program and you qualify, a member of the virtual heart failure team will contact you. If you’re unsure if your doctor offers HekaHeart, ask them for more information.

  • Your HekaHeart team will schedule virtual appointments with you - either through phone or video. You’ll also have a dedicated phone line to call or text your HekaHeart team anytime you need them.

  • Most patients stay in the program for 12 months or longer, but you can opt out at any time.

  • Yes! Your virtual heart failure team works alongside your existing provider to ensure seamless, coordinated care.

  • Absolutely! Please continue to see your heart doctor as you normally would. Your virtual heart failure team can help coordinate appointments as needed.

  • The cost of the program depends on your insurance coverage. For example, Medicare Part B patients typically have a 20% copay for care management and remote monitoring, which is around $20–$60 per month.

  • Coverage varies based on your insurance plan. Many insurance providers cover aspects of the program, such as remote monitoring and care management, but costs may depend on your specific benefits.

    Don't worry—we'll review your coverage and provide a breakdown of expected costs before you start the program!

  • You’ll have weekly phone or video check-ins at the start of the program, with helpful reminder texts in between. As the program progresses, formal check-ins become less frequent, but you can always call or text your team anytime.

  • Your smart scale and blood pressure cuff will be shipped directly to your home—no setup required! Your care team is available to assist with any questions, and you are not charged for the devices.
